Position Overview

The Sr Nurse Case Manager is responsible and accountable for coordination of services for Medicare client members referred for complex case management. This role involves collaboration with healthcare providers, patients, and families to develop and implement individualized care plans.

Key Responsibilities

• Assess patient needs and develop comprehensive care plans
• Coordinate care among multiple providers and services
• Monitor patient progress and adjust care plans as necessary
• Educate patients and families about health management and available resources
• Maintain accurate and timely documentation of patient interactions and care plans
• Advocate for patients to ensure access to services and resources
• Collaborate with interdisciplinary teams to enhance patient outcomes
  
  

Qualifications

• Current Nursing License
• Proven experience in case management
• Registered Nurse (RN) designation
• Strong communication and interpersonal skills
• Ability to work within a team and independently
• Knowledge of healthcare regulations and compliance
• Bachelor's degree in Nursing or related field preferred
